$6.00 on (1) 700ml O’Shea’s Irish Whiskey per bottle you must have:
- Original certificate, including your signature and date of birth to certify that you are 21 years of age or older.
2. Fill in the UPC# in the space(s) provided on this form.
3. Original, dated, store-identified cash register receipt(s) with price(s)
circled for purchase(s) made during valid time period of this offer.

4. Limit one request per name, address, or household. Additional requests will not be honored or returned.
